Grace Rehabilitation Center of Vero Beach’s skilled nursing services are oriented around provision of specialized care for recovery after hospitalization. Our dedicated team helps residents and family members feel at home while receiving individually focused healthcare. From nursing care and nutritional support for recovery, to the activities of daily living, we focus on an innovative, comprehensive interdisciplinary approach.

Skilled nursing is rehabilitative care provided to Medicare beneficiaries or individuals covered by private pay and some health insurance plans. Residents requiring continued long-term care may qualify for Medicaid benefits.

Stroke recovery

The recovery process after a stroke can be very overwhelming. Strokes can lead to major changes in a person’s physical and cognitive abilities. Here at Grace, we take a Whole Person Care approach that involves our entire interdisciplinary team while helping patients recover from a stroke. Our highly trained therapy and clinical teams assist stroke recovery patients regain as much of those lost abilities as possible, while our social services, dietary and activities departments focus on the rest of the patient’s well-being. We want to help the patient reach their highest possible potential by developing strategies to compensate for any challenges, and also help families adjust and plan for life after a stroke.

Our clinical team has spent years building a program to help patients manage their acute CHF symptoms. Our program involves constant monitoring of symptoms of the disease, as well as good communication with physicians to quickly react to changing conditions. Our center has a very high success rate of managing the disease process and preventing unnecessary trips back to the hospital. And, just as important as disease management in our facility is, working with the patients and their families to set them up for success in continuing to manage the chronic disease process after going home is essential.

Infections can lead to many problems both clinically and mentally for patients. Our clinical team is equipped to receive information faster, and therefore can react promptly to infection changes. We can help to keep the fight going against the infection while assisting the patient through physical rehabilitation with a goal to restore the prior functional level as quickly as possible.
